Securian Farm Credit Ag-Related Death Claim Request Process

How do you file a claim?

  • If you receive a call from a Farm Credit Branch Agent

*The Farm Credit representatives serve as agents for Securian and CAN inquire about coverage information and beneficiary information on behalf of the members.

  1. Complete the death claim form request: Securian O-S And Farm Credit Claim Form 12.26.17.docx
  2. Print screen shots of the coverage search page of FCLifenet: 
  3. Determine if a Notice of Death is needed:

 

  1. If the Farm Credit Representative will need to provide a Notice of Death 
    • If the Farm Credit representative needs assistance with locating the notice of death form on FCLifeNet, you may walk them through our FCLifeNet website. The Notice of Death forms are found under the Maintenance Form tab.
    • The CSR should ask the branch for an email address of a contact person so when the admin team starts the claim process, they can email them a copy of the Notice of Death to complete.
    • You may also offer to email them the Notice of Death form that is attached: fc life net NOD.pdf
    • If they request a W9 form please use attached form: w9.pdf
  2. Email billingworkflow@selmanco.com to notify of the death. Print a copy of the email to ImageNow.
  3. Place a detailed note on the InsPro record indicating that you sent the required documents to the research box. Also, notate the date of death on the record in InsPro.
  4. Send the death claim request via email to Securian Farm Credit email box, farmcreditresearch@selmanco.com including the DEATH CLAIM WORKSHEET & FCLifeNet screenshots. Label the subject line as follows: Insured’s Full Name-ID Number-Securian-Death Claim.
  1. Please print off hard copies of the claim packet request form, along with the 2 screenshots from the coverage search page of FCLifenet. Place hard copies in ImageNow bins that day. 

*Upon receipt of the Notice of Death, Katrina Lewis will send the complete claim file to Securian. 

  • Mid America is the only branch that we fill out the NOD for them. We contact Annette (Annette.Cook@SunStreamServices.com) to see if there is an outstanding loan or not because Mid America no longer has that information at the branch level.
