° Employees are given a rate chart with the application. It is included in the enrollment kit.
° Payments can be EFT (upon request; monthly, quarterly, semi-annually, or annually) or premium notice (quarterly, semi-annually, annually). We will use this Aetna EFT Authorization Form. APO Form - MQSA (1104APO).doc
° EFT debits will occur on the 15th or the 24th. If the first EFT attempt on the 15th fails for insufficient funds, a rebill will be performed on the 24th. If the first attempt is made on the 24th and it fails, a rebill will be attempted on the 5th. After two failed attempts, the billing mode will be changed to quarterly direct bill and an invoice will be issued. The initial invoice will request enough premium to bring the insured’s paid-to date to the next calendar quarter.
° Currently, McCamish is withdrawing EFT payments on 16th or last day of the month. SelmanCo. will be changing those EFT billing dates to the 15th OR 24th. Insureds can later change their billing date to the 5th, 15th, or 24th by completed an EFT Authorization form.
° Adult rates increase gradually in five-year age bands (ages ending in 0 and 5). Child rates will not have an increase band.
° Age-related rate increases take effect on the January 1st next following the insured’s attainment of the specified age.
° Rates are tobacco-specific. Aetna has advised that nicotine gum and smokeless cigarettes are NOT considered tobacco products.
