No. The provisions of the Portability plan are outlined in the group employer booklet. 

We do issue an Insert A (similar to a schedule page) to outline the amounts and effective dates of the ported coverage. CSR’s will email certificates@selmanco.com to request for a copy of the Insert A if the insured would like a copy.
