The member will need an Increase Kit. 

The kit consists of a quote letter and the Medical Questionnaire Enrollment Form (varies by state). The cover letter provides the rate for the member’s current age, per $1, per mode. 

In this example letter: Aetna LTC -- UW Increase Letter.doc, the member is quoted $4.20. This is the extra premium the member will pay for each dollar added to her DBA. The member pays quarterly, so the quote is also per quarter.
