What to do if you receive a call to reprint a 1099 tax form?

• Call 800-293-8607
• Email gbd.sickpayw2@thehartford.com 
  1. Include insured / beneficiary’s name, mailing address, and social security number
• Copy can be emailed but must be password protected so user will receive 2 emails (1 password / 1 document)
• Approx 5 business days
