Claim Overview Tab: How to find the Plan Type: Service Model OR Disability Model

To find out if the insured has a Service Model OR Disability Model
- Stay on the Claims Overview Screen
- Look for Claim Plan Code
- Click on the I icon where it states: FuturePur- Future Plan

The following screen will then populate
Look in the: Claim Processing Mode to see which circle is checked off
- Service OR Indemnity
  - Service= Service Model
  - Indemnity= Disability Model
