How to file Aetna Conversion DXC claims going forward:

1. Complete the attached Hartford Claims Worksheet (they will no longer be completing the claim over the phone, they will just be providing us information so we can send them out a claim packet).

2. Send the completed attached Hartford Claims Worksheet to ImageNow under the Aetna Conv Drawer

3. Advise the caller they will be receiving a claim packet in the mail to complete and send back to us with a certified death certificate

4. Our claims office will mail the claim packet to the beneficiary on your behalf, and they will scan a copy of the claim packet to ImageNow for your access in case the caller needs help with the claim forms.

5. Beneficiary will complete the claim packet and mail it back to our office with certified death certificate

6. Our claims office will process and pay the claim

Benefit information should not be disclosed to anyone until after the claim has been processed.   Even if there is a funeral home assignment on file, please advise that no information can be released until the claim has been processed.

**Some policies have a settlement options, where the beneficiary can select to have the claims paid gradually instead of a lump sum** See attached Settlement Option Form that will be mailed to the beneficiary with the Claim Form only if the policy requires it. Settlement Option Addendum.docx
